When to Post to PCC
Once it has been decided that a referral will be admitted to a facility, you should post the referral to PCC. This action creates a pending patient in PCC and includes basic demographic information.
How to Post a Referral to PCC
- When a referral is in the Admitted or Accepted status in at least one facility, the "Post to PointClickCare" action will be available when you click on the “3-dot” button at the top right of the referral page.
- Click on "Post to PointClickCare". A pop-up window will appear.
- Choose the facility where you want to post the referral as a Pending Patient.
- Click the "Post" button. Note: You can only choose facilities that are on the referral and in the Admitted or Accepted status.
- PCC will attempt to find a match for the patient. If a match is found, you will be prompted to either match this referral to the existing record or proceed with creating a new Pending Patient.
Where to Find the Patient in PCC
After posting the referral to PCC, you can locate the patient in PCC under the External Pending list specific to the chosen facility. In Referral Tracker, the referral page will show "PointClickCare Status Pending in [facility name]" (you may need to refresh the page to view this).
Please note, PCC only allows very basic data to be sent over with the referral, so you"ll still need to fill in most of the fields there manually.
Accepting Pending Patients in PCC
On the left of each pending patient, there is an actions button where you can accept the patient.
After Acceptance in PCC
Once the patient has been accepted in PCC, Referral Tracker will update with the PointClickCare Resident Number. This number will be displayed prominently at the top of the referral page.
Uploading Documents to PCC
Once the patient has been accepted to PCC, you can upload referral documents to PointClickCare.
- Click on the referral’s Documents tab.
- If there are documents attached to the referral, you will see the "Upload to PointClickCare" button.
- Click on the button and select the documents you wish to upload.
- For each document, you will need to select a PointClickCare category.
- Click the Upload to PointClickCare button to upload.
Please note: PCC has a 15Mb size limit per document. Documents that exceed this amount can not be uploaded to PCC.
