Creating a New Grievance

Create new grievances on behalf of the Residents or their Representatives.

  1. On the Home page, select the Grievance Log App GL_App_Icon.png, or click on the menu button on the top header 👇 and select Grievance Log from the main menu.

  2. From the Grievance Log Home page, click on the Add_New_Grievance.png button.
  3. The Grievance form will open, and you will see four sections on the left-hand column:
    • Section 1- Grievance Details - Input the background information of the grievance.
    • Section 2- Follow Up - Enter what actions were taken and what additional information was found out.
    • Section 3- Resolution - Provide the details of how the grievance was resolved.
    • Section 4- Associated Files - Upload any documents that are part of this grievance.
  4. The center section is where you will enter the relevant data, and it will change based on the section you are currently in.
  5. The right-hand column displays a summary of the information you have entered for the grievance.
  6. Begin by filling out the Grievance Details section.
  7. Once you have completed all the required fields (displayed with an Asterisk*), you can save the grievance, and it will be created.
  8. Continue the Grievance Log process until completion, by filling out the Follow Up and Resolution sections.
After the grievance is saved, you will not be able to edit the Grievance Details and Resolution sections unless you have been granted the "Edit Grievances" permission under Settings > User.

 

Public Form: Grievances can also be submitted from people that don't have access to SNF Metrics via a public form. Ask your organizations admin or DON on how you can access the form.
